Do you know a parent who has been rendered  disabled  due to the following:

​

  • Diabetes

  • Gun Violence

  • Kidney Disease

  • Lung Cancer
     

If so, please have their parent or guardian fill out our application.

Application Guidelines

  • Children must be between the ages of 5 and 12 years old.

  • Applications are only accepted from parents, grandparents, or legal guardians.

  • All applicants must provide proof of debilitating condition with the application.

  • Applicants must provide Your Special Day with a photo of the referred child.

  • Incomplete applications will not be considered. Please provide and verify all necessary information. One application per child.

Upon submission, all responses for a referred child will be sent within two weeks of the date of submission. Completion of an application does not guarantee approval.
We are currently seeking families living in the following cities:

 

  • South Los Angeles

  • Watts

  • Compton

  • Inglewood

  • Gardena

  • Hawthorne

  • Huntington Park

  • and South Gate.
